3. Job Title (list only oneprimary job-wehre the bulk of your experience
is)
4. Union(s)
5. Website (if applicable)
6. Representation (telephone and website, if applicable)
7. Bio (no longer than 4-5 sentences… consider including education, key projects,
career highlights, recent projects, awards and please write in the 3rd person
tense)
8. Membership Type (general or associate)
9. Favorite Ice Cream Flavor
10. Contact Info. (phone, fax, email, address)
